Appointment Late Cancellation
Please fill out our Appointment Cancellation Request if you are cancelling within 24 hours of your appointment time and date.
PLEASE NOTE: Cancellations that occur WITHIN 24 HOURS of the scheduled appointment will be considered a late cancellation. All unexcused late cancellations will result in a $85 LATE-CANCELLATION FEE.
